Following are the broad classified causes of human infertility -

Female Factor -

  • Blocked tubes
  • Tubal diseases like tuberculosis
  • Inability to ovulate ( Maturation of female egg)
  • Premature ovarian failure

Male Factor -

  • Low sperm count / Absent sperms or very low motility
  • Hormonal or genetic problems
  • Testicular failure
  • Immunologic causes of infertility
  • Unexplained infertility
  • IVF -

    Invitro fertilization or test tube baby ( as it is commonly known as ) is a procedure in which the union of the egg from the female and the sperm from the male is carried out externally, in a laboratory rather than it occuring in the fallopian tube of the female; where is occurs naturally. This fertilized embryo ( product of conception ) is then placed in the mother's womb where is grows naturally like any other normal pregnancy to nine months of gestation.

I V F Pre-Treatment Selection and Management

Selection criteria for patient

Following are the various causes of infertility that can be treated by ART -

Basic Health Check-up

  • Absence, damage or blockage of fallopian tubes due to various factors (IVF)
  • Male factor infertility (ICSI-IVF) [Sperm count is low].
  • Unexplained infertility with normal reports.
  • Unfavourable cervix.
  • Endometriosis (IVF)

The presence of a uterus and at least one functioning ovary is a prerequisite for IVF and GIFT.

IVF may be difficult if not impossible with severe decrease in the male patient's sperm count. In most cases of severe male factor, normal fertilization rate can be achieved through Intracytoplasmic Sperm Injection (ICSI). In the presence of one of the above female factors and markedly abnormal semen analysis, IVF can be combined with the use of donor semen alone or in combination with the partner's sperm.

First Visit and Screening Tests

  • Past medical records, especially operative reports, results of semen analyses and tests of ovulation (FSH and progesterone tests) should be deposited in the initial visit at Ashvini Hospital's IVF Center.
  • After the basic examination of the couple, we advise the following.

1. For the female

  • Blood test including hormonal evaluation llike FSH; LH, TSH and prolactin ( Hormonal study for ovarian functions )
  • Complete blood count, Blood sugar, HIV, HbsAg, VDRL.
  • Baseline Ultrasonography.
  • Tubal testing by Hysterosalpingography or by Laproscopy.

1. For male partner

  • Semen Analysis.
  • Baseline blood test like HIV / Hbs Ag
  • At the time of initial visit, a trial transfer is done. It is normally painless and allows to perform the actual embryo transfer with as little irritation as possible. It is done by passing a fine plastic tube into the uterus (womb) in order to assess the direction of the passageway through the cervix (the neck of the womb) and to measure uterine depth.

    If one or both tubes are closed at the distal end near the ovaries, forming a hydrosalpinx and then removal of the damaged tubes may be advised, as closed tubes lower pregnancy rates even with IVF.

IVF Simplified

IVF stands for In-Vitro Fertilization. In this process, the female partner's egg is fertilized outside the body with the husband's sperm and resultant embryo is placed in the uterine cavity. In this activity the fallopian tubes are bypassed. Fallopian tubes in female body are the place where normal fertilization happens.

This technique is carried out when following cases are detected,

  • Fallopian tubes are blocked or are unhealthy,
  • Sperm count is low,
  • Endometriosis,
  • Any other unclear causes of infertility.

The female partner is given medications to increase the number of eggs. To study the hormone levels, the development of the egg, technically called as Follicle is carefully monitored with regular tests of blood and ultrasonography. These activities are carried out to improve the success rate. At the right time, when the egg is fully matured, egg collection activity is done. The mature egg is then transferred into a test tube and kept in the incubator. Favourable conditions for growth are maintained as the test tube is placed in an incubator.

The male partner is advised to produce semen sample. Healthy motile sperms are taken from the semen sample. These sperms are placed in the test tube where the female partner's mature egg was placed earlier. The egg is observed for next twenty four hours. This fertilized egg is known as Zygote. It starts developing into 2, 4 and 8 cells. This goes on till it develops into Blastocyst, a fluid filled ball of cells. 2 - 3 embryos at this stage are placed in the uterine cavity of the female partner. To help the implantation process the female partner is given hormonal medication. 2 weeks after this implantation, the pregnancy is confirmed. The pregnancy is treated like a normal pregnancy thereafter.

Getting Familiar With The Lab - Lets Take A Tour IVF Laboratory - Its Equipment

Operation Theatre -

The Operation Theatre and the Laboratory are adjacent to each other. Both these communicate with each other through a window / hatch as well as intervening door which is kept closed during most of the procedures. This ensures that the eggs which are being retrieved from the patient or the embryos that are being transferred back into the patients womb are not exposed to the detrimental effect of fluctuating environments. Our theatre is equipped with the latest anesthetic machines, oxymeters and cardiac monitors to constantly monitor the patient. The eggs are aspirated using well-crafted disposable CCD needles from France, with the help of high quality Vaginal Ultrasound. The suction force required to aspirate these eggs are generated by a special digital suction machine from Cook, Australia.

IVF procedures requires easy access to a incubator and maintenance of accurate temperature and CO2 level. At Ashvini IVF Center we have a high end incubator. This sophisticated incubator maintains a sterile, womb like environment with osmolarity accurate temperature, humidity and pH.
